Who Qualifies?

Care Recipient

Lives in Georgia
On Medicaid or Medicaid-eligible
Age 21 or older
Needs help with day-to-day activities
Requires 5+ hours of care daily (nursing home level)

Family Caregiver

Age 18 or older
Lives in the same household as the care recipient
Family member — biological or by marriage
Passes a satisfactory background check
Spouses and legal guardians are eligible
